The Outdated ‘Watch and Wait’ Approach to Adolescent Scoliosis: A Modern Perspective

For many years, the “watch and wait” approach has been a common recommendation for adolescents diagnosed with scoliosis, particularly when the curvature is measured at less than 20 degrees. In such cases, children are typically advised to engage in general strengthening exercises while their condition is monitored over time to assess any progression. If the curvature worsens, only then are they referred for bracing or surgery.
However, as chiropractors specializing in spinal alignment, we believe this approach is both outdated and potentially detrimental to a child’s health. Waiting for scoliosis to worsen before taking corrective action can significantly limit the potential for non-invasive treatments. When we identify scoliosis or a curvature that signals a high likelihood of progression, early intervention is key. Whether the goal is to correct, improve, or halt the progression of scoliosis, addressing the condition immediately can yield significantly better outcomes for both children and adults.
Today, we’re highlighting two cases that illustrate the power of early intervention in scoliosis care:
Case 1: 
A 19-year-old patient came to us after being diagnosed with a 13-degree spinal curvature at age 11. At that time, the family was advised to “watch and wait.” Unfortunately, by the time she presented to SPC for treatment at 19, her curvature had progressed to 30 degrees. This degree of scoliosis is in the realm of surgical consultation. Fortunately, this family found our clinic, and after just one year of chiropractic care, her curvature was reduced to 19 degrees—avoiding the need for surgery.
 
Case 2:
A 10-year-old patient came to us for a routine wellness check and was found to have a 14-degree curvature, predictive of future scoliosis progression. With consistent care over the next eight months, her curvature improved to 10 degrees. By the time she reached 15, her spine had corrected to a 0-degree curvature.
 
These cases highlight the benefits of proactive care in scoliosis management. By addressing spinal curvature early, we can often avoid the need for more invasive interventions later on, offering children and their families better outcomes and peace of mind.
